Request to delete personal information
You may ask SmileKonnect and your dental team to delete personal information about you. Submitting this form does not erase your information automatically. Dental professionals are often required by Ontario law and professional standards (including Royal College of Dental Surgeons of Ontario recordkeeping expectations) to keep clinical and related records for many years.
Retention notice
Where retention rules apply, some information cannot be deleted yet. A dentist or clinic representative will assess your request. You may also email smilekonnect@drrameshagrawal.com.

What happens next?
1. Request recorded
Your submission is logged for follow-up.
2. Dentists notified
Active dentists on SmileKonnect are notified so they can review the request.
3. Legal and professional review
The dental team assesses whether deletion, restriction, de-identification, or continued retention is required.
4. Outcome
You may be contacted about the outcome. Review is often expected within about 30 days; complex clinical record issues can take longer.
